Agency Connections Questions (Q18, 18a)

Agency Connections is the last section of questions. Here, we want to know if the student connected with an adult service agency. This does not necessarily mean the former student received services.

In the year since leaving high school, did you connect with an adult service agency?  Examples include: Division of Vocational Rehabilitation, Developmental Disabilities Administration, Disability Support Services, WorkSource, a mental health provider, Job Corps.

Question 18 asks if the former student connected with an adult service agency in the 12-month period after leaving high school. If the respondent isn’t sure, the interviewer might give them some examples, like Division of Vocational Rehabilitation, Developmental Disabilities Administration, or Disability Support Services through a college.

For question 18,

  • If Yes is selected, question 18a appears.
  • If No or Don’t know/declined to answer are selected, continue to Section VIII: Additional Comments.

Question 18a asks for the names of the agencies the former student connected with. The interviewer can select multiple answers. It may be helpful if the interviewer reads some of the options. If an agency the former student connected with is not listed, select Other and provide the name in the text box.
