Module 3 Summary
Review: Verifying the Leaver Survey List
- Leaver Verification (LV) TSF2 User Role
- The LV user role should be assigned to someone with access to student-level data.
- DM and LV users are the only accounts that have permissions to edit the Leaver Survey List for their assigned schools.
- Prepare to Verify the Leaver Survey List
- Ensure you have a copy of your school’s leaver demographics and contact information from the previous school year.
- Ensure you have access to the TSF2.
- Review and Edit the Leaver Survey List
- Each school’s pre-loaded Leaver Survey List (and former student demographics) must be reviewed and edited as needed to match school/district records.
- Leaver criteria for 2025 Survey: former students ages 16-21, permanently exited high school in the 2023-24 school year, and were receiving special education services at the time of exit.
- Guidance on Deleting Leaver Surveys
- Surveys can be deleted if the student’s age, exit year, and/or IEP status do not meet the leaver criteria.
- Surveys should not be deleted for former students who dropped out, were enrolled but did not attend, transferred without confirmation, or are deceased.
- Verify the Leaver Survey List
- Schools with and without leavers must verify their Leaver Survey Lists.
- Survey phone calls cannot begin until the Leaver Survey List is verified as accurate.
- Any time a Leaver Survey List is edited after it’s been verified, it must be re-verified.
