Review: High School and Beyond Plan Alignment

• IEP Transition Plan and the High School and Beyond Plan (HSBP)
  • IEP is a federal mandate. HSBP is a state graduation requirement.
  • Although the documents are different, many components in the IEP transition plan and HSBP align.
  • T-Folio is a free, online transition curriculum that can be used to support the development of both the IEP transition plan and the HSBP.

• Ask about and discuss how the HSBP’s and the transition section of the IEPs for students with disabilities coordinated and aligned at your school.
• Record any key takeaways from the conversation in your workbook.
